2007 Halloween Am Jam Pre-Registration

 

This year we've made pre-registration even easier. Just follow these simple instructions.

 

1. Download the contest registration form and the waiver ( for skaters new to the park ), and fill them out.

2. Send the forms, as well as payment to Kona Skatepark.
....Option A : Mail check or money order to Kona Skatepark, 8739 Kona Ave, Jacksonville FL, 32211
.....Option B: Fax or mail credit card payments to (904)725-7881, or above address.
